Is there an existing issue for this?

Describe the bug

The Contact Us Page is not appropriate and it is degrading the user experience the Contact Us page must look appealing to the user as every user visits to Contact Us page. Contact Us Page can take more data about the user which could help later for data analysis.

Expected behavior

Contact Us Page will take some more user input and details. The styling of the page will be enhanced so as to improve the user experience.
