terrain database with airports and state_insets

Hi,
I'm very impressed with this work and am getting it going for tiles in British Columbia, Canada.

There is reference in labels.mml to a terrain database with airports and state_insets but I've yet to find a reference as to how to create and populate this database.

Any hints?
