Document remediation tools (fix, not just detect)

Summary

Add MCP tools that can programmatically fix accessibility issues in Office documents and PDFs, not just detect and report them.

Why

Currently the agents detect issues and recommend fixes, but the user has to manually open the document and apply changes. Remediation tools would allow agents to fix issues directly — adding alt text, setting document titles, fixing heading structure, adding table headers.

Scope

• Office document remediation (DOCX, XLSX, PPTX) via XML manipulation
• PDF remediation where structurally possible (metadata, tags)
• Batch remediation across multiple documents
• Dry-run mode to preview changes before applying

I think this issue should be revised to also include epub files as well. Since they are html, and css, a lot of the work is already in place.
I have already verified that claude can work on epubs.
