Feat: Customize name of folders and file outputs

Add the ability to customize the template of the created folders and file names.

Ex: %(channel)s [youtube2-%(channel_id)s]/%(upload_date>%Y_%m_%d)s %(title)s [%(display_id)s]/%(upload_date>%Y_%m_%d)s %(title)s [%(display_id)s].%(ext)s

In Plex I am using:

This allows downloaded videos to be organized by channel, like a TV series, and the current naming convention is not compatible with the scanner/agent.

This would be a great feature for organization. I'm having similar issues where Plex will put all the channels into a "Collections" tab, but all the videos under the collection will always start with the "[Channel Name] - " and this creates a lot of problems when trying to find the video file that you want to play.

I have a few tickets ahead of this one, but adding more user-defined flexibility is a good idea.
