Describe the bug:
Users can submit a blank request via the Grade Scale Edit form.

Steps to reproduce:

  1. Open the Grade Scale Edit form.
  2. Leave all fields blank.
  3. Submit the form.
    4.goto my task approver accept the request

Expected Behavior:
Form should prevent submission of blank requests.

Actual Behavior:
Blank requests are accepted from my task approver
