Underage membership application

The membership application form needs to have a checkbox: "I am under 18 years old" or year of birth field. For underage applicants, a guardian's contact details (phone number should be sufficient) should also be collected.

Is it necessary? There are so few applicants and we can figure that out from the year of birth for everybody except those who are turning 18 that year. We can ask it manually.

If it's important then we could make a hidden field which is shown only if user inputs year of birth which is less than or equal to 18 years from this year.
