Add the ability for therapist to edit the following fields:

So this will essentially look and contain the same functionality as the registration form? Currently there are two separate forms -- one RegisterForm and one ProfileEditForm -- should we just merge the forms together.

Task list for this issue (which I assigned to myself).

Both parents and therapists should be able to edit their:

  • Display name
  • Email address
  • Phone number
  • Postal code

Therapists alone should be able to edit their:

  • ABA training course status (completed or not)
  • Certificate of Conduct status (whether they have it or not)
  • Date of Certificate of Conduct receipt

I don't really think gender needs to be editable, and that there should be a different form for password changes. The password change form could be a new page, or just a different part of the same page, but I think we need to make it super-clear that users are changing their login creds.

- Postal code
Added for therapists:
- ABA training course status
- Certificate of Conduct
- Certificate of Conduct date of reciept (MUNComputerScienceSociety#93)

Updated tests as well.
