Pre-assessment & Assessment Date

Date should be a part of the assessment, not something at the top of the page...

The date is used as a way to group the assessments together into a visit. I think we need to either drop the idea of a visit that groups them together and come up with a different way to group them or drop the grouping all together and just have all the assessments from a facility in the wizard.
