docs: add skill usage examples to README

Context

The README describes each skill's purpose but doesn't show example output or common usage patterns. New users would benefit from seeing what a typical /sw-design or /sw-verify session looks like.

Suggested scope

• Pick 2-3 skills (e.g. /sw-design, /sw-verify, /sw-learn)
• Add a short usage example or sample output snippet under each in the README or in a new docs/examples.md
• Keep examples concise — 5-10 lines each

Helpful references

• skills/ directory contains the SKILL.md files with goal descriptions
• README.md has the current skill table

I’m new to contributing, and this looks like a great way to get familiar with the project. I’ll focus on adding concise usage examples for 2–3 skills as suggested.
