Dynamically update meta title

Update the meta title depending on the routing state, so it's easier to use multiple tabs/instances of the application.

Criteria
* default: SQLite Remote
* when just a database is open -> '{{data source}} - SQLite Remote' e.g. 'My SQLite server - SQLite Remote'
* when a table is open -> '{{table}} ' {{data source}} - SQLite Remote' e.g. 'Employees | My SQLite server - SQLite Remote'
