BUG:Contact From showing twice in the website

What happened?

The contact us part is showing twice. Once with only with the headline and with no sub heading given and other with proper contact form.

I'm gonna fix it with removing the upper one and will also redesign the working contact form. Moreover when clicked to the contact button in the header it redirects to the wrong one.
