When an app is updated, it's icon becomes "default android" icon. It happens to homescreen and drawer.
Rebooting the device fixes them, but they should be refreshed without rebooting.

you mean other apps or ours? Do other launchers work well?

It happen with playstore apps and fdroid apps; any of them.
Other launhcers work ok
