remove --local

**Describe the bug**
We don't need this flag any more. Lets remove it.

There are many code paths that differ based on whether local is true so we need to go through them all and remove the dead code.

- vscode handling can be removed
- wskdebug package download, and all deps can be removed
- all checks behind `aio app run --local`
