Separation of evaluation forms

Description
Evaluation forms for faculty and Faculty interns should be different from that of students.
The evaluation forms students will use to evaluate peers should not conflict with that of faculty.

Enhancements
Create an option for users to select :
Is the user a faculty, faculty intern or student?
Enable users to enter information
Take the user to a page where form is displayed

Test Criteria
Authenticate users by email address and password
Validate information
