Update README.md - Fixed Twitter follow button

Adjusted Twitter follow button/badge to actually redirect to our Twitter profile instead of just opening the badge image.
Embeded the code created via https://shields.io/badges/x-formerly-twitter-follow into a markdown link, as suggested here:
