[Bug] Clicking "regenerate insight" button on focus card doesn't work

Description

Clicking that button says "Without specific lab values provided, no marker can be named as the most important finding." Even though this is after importing labs.

Fixed — improved the focus card system prompt to work better with local models (clearer constraints, explicit word limit). The regenerate button now produces more reliable one-sentence insights.

Fixed in v1.6.1.
