Add refresh rate

Since the proxy throws away the refresh interval, it's up to the app to decide when to refresh. We should add a setting.

Or do something janky and send a preamble in front of the BMP bytes… e.g. the very first byte is the refresh interval in minutes, then the BMP bytes follow.
