NarType_tab and NarExplanation_tab have been used somewhat interchangably (though NarExplanation_tab tends to hold more extended note type information).

PROPOSAL: Keep both fields and better define their usage:

• Standardise values in NarType_tab
• Remove the lookup list from NarExplanation_tab
