Copilot putting reasoning / explanations of commands in the command as comments

Describe the bug

After enough time iterating on a single turn, Copilot likes to put comments at the start of commands that it runs rather than in actual messages, so they get hidden when I don't have output expanded.

0.0.419

Steps to reproduce the behavior

1. Have copilot (Opus 4.6 1m context) run a long sessions where it iterates on its own running commands back to back
2. It starts to put comments in the commands instead of in messages
3. It can be prompted back, but eventually returns to putting comments in commands

Expected behavior

It should put its reasoning and explanations of the command in messages for me to observe.
