Stats and disable state not pulling on Mac. Temporary disable will work, but the icon won't change back.

Hmm, showing stats has been working fine for me on macOS.

Can you access the Pi-hole web interface in a browser on the same machine?

What do you have in the "address" field when configuring the action?
