Elements in the Serial "Work" form that have neither a text entry box nor a button that one can click on to look up or enter data:

Form or work
Place of origin of the work
Subject of the work

Elements with the same problem in the "Instance" form:

Title in more than one form
Later title proper
LC classification

This is fixed (I think...I'm not sure about Elements with the same problem in the "Instance" form)

Please clarify if this is still a problem.
