Print instead of writing to file

When -o is not given, print converted contents to stdout. This is backward incompatible, because currently Subconvert will try to write to the file with some default extension.

This could also remove default extensions from formats spec.

This also makes easier writing some integration tests (i.e. the ones launching the whole Subconvert in console mode).
