Substream backpressure #1
Description
Right now substreams pretty much just do firehose mode, limited only by tunnel-level backpressure. Instead, if a substream is not being read by one end, the other end should stop pushing data. See notes on the early protocol sketch for a simple way we could accomplish this (HT @kevinmehall).
