Improve module management

• Add toggle for public/private module to UI
• Allow user to select repo when creating new module in UI
• Use current repository when creating new module via CLI
• Specify "include" globs in module manifest; use them when packaging module
• Provide something like nullstone apply --publish that will publish the current module and apply in Nullstone
