myHealth: add profile info to the header of each exam

Should say something about who the test is regarding in the header next to the date , certainly if we were to export this.

@jessewashburn I wonder if there should be a feature where you have the option to give your examination a custom name if you would like. If not the default name could just be like the username and the date separated with an underscore or something.

I think this is a great idea.
