Certified Copies : Multiple Fields Showing Blank in Marriage Certified Copies

Bug Description:
Multiple fields in marriage declaration show blank in certified copies.

Steps to reproduce:

1. Login as RA/local reg/ national reg.
2. Navigate to ready to print workqueue.
3. Download any marriage record and click on 'Print'.
4. Select 'marriage certificate certified copy' template for certificate printing.
5. Compare the values from 'view' page with the certificate.

Actural Result:
Following fields show blank in the marriage certified copies -

• Marriage Registration No.
• Last name at birth (for Groom)
• Last name at birht (for Bride)

Expected Result:
All fields should show correct values.
