Combine link and problem name and location columns on all three course-level dashboards

User Story
As a course delivery team member, I want to combine the problem link and name and location columns on the Course, Individual, and At-Risk Learner dashboards, so that there are less columns to sift though when looking for data on these three course-level dashboards.

Acceptance Criteria
Given I have access to the Course, Individual and At-Risk Learner dashboards
When I view the following tables on the three course-level dashboards dashboard:

• On the Course Dashboard and At-Risk Learner Dashboard:
  • Engagement/Problems: Problem Attempts and Results table
• On the Individual Learner Dashboard:
  • Problems: Problem Results table

Then I see that the name and location and the link columns have been combined to one name and location table with the name and location text now being a link to the problem in the course (Note: this change should only be made if when made the user is able to still use this cell as a clickable filter when the name is changed to a link).
