Alt alert style and app error when removing a team member

As a custodian or an advanced user, if you remove a team member, you see the default browser alert, rather than the 'are you sure' pattern. When you confirm the removal of a team member you get an application error.

Steps to recreate:

1. Login as a custodian or advanced user
2. Select 'Manage team' from the nav
3. Click the 'Change' link next to a team member name
4. Click the 'Remove from team' link
5. When you get the browser alert click 'Ok'

Agree this is an enhancement that can wait for next iteration, but think there is a bug in the current implementation that does need fixing before we ship (the application error if the user confirms in the browser prompt)

The bug has been fixed. Not style improvements have been done.
