Refactor pdf-struct-dump.p6 to use LibXML. Add HREFS for links? #3

pdf-struct-dump.p6 is doing a simple single pass and directly writing XML to stdout.

However, as prerequisite to add links (forward and backward). This needs to first be restructured to save to an intermediate DOM.
