Go to https://github.com/rebtd7/FF13Fix/releases
Download the latest version (e.g. FF13Fix_164.zip) and extract the zip.
It should contain a folder, and inside it the d3d9.dll

It might be because Windows Defender flagged d3d9.dll - It did for me. If that's the case, just manually approve it.
