MySQL with access denied

I tried to use BlueStats to showcase Statistics of the server in our website. However, after correctly setting up everything, I end up with a blank page. I turned on the debug mode and I got these errors.

Also, unrelated, the admin page keeps associating to "Lolmewnstats" when I have Statz implemented. Some of the pages there, like Mcmmo, Installation Settings, and URL's, do not work.
