Feat: Dynamic Tool Registration / Discovery #151

I've been thinking about adding something like dynamic tool registration and discovery a bit lately, this could help when a large number of tools, or tool parameters are enabled.
Today I saw Anthropic released a new approach to tooling discovery ('advanced tool use') which I think summed it up pretty well and had some interested patterns. I could look into doing something similar - perhaps also solving for #145: https://www.anthropic.com/engineering/advanced-tool-use
