add tag for each forwarded port

Currently, the output shown when running the tunn status command is as follows:

Tunnels Ready

[api]
    3000 ➜ 3000 [active]
    4000 ➜ 4001 [active]
[db]
    3306 ➜ 3306 [connecting]
    5432 ➜ 5432 [active]

What I expect is to be able to label each port — for example, indicating which app is listening on that port:

Tunnels Ready

[api]
    3000 ➜ 3000 [htpp, active]
    4000 ➜ 4001 [grpc, active]
[db]
    3306 ➜ 3306 [MySQL, connecting]
    5432 ➜ 5432 [Postgres, active]
