Reference to cob_index version 0.4.0 release

• Trying a new versioning system to reference the version of cob_index that we expect to use with this release.

This should make environment releases easier to think about since there is a dependency both on the schema and the version of the indexing tool.

Fix solr.ICUFoldingFilterFactory analysis not applied to all search fields.
