Add Apply Button

There should be a button on the top right that says "Apply". It should link to a Google Form or equivalent that the HOTH directors are using to track signups.

The button should be grayed out when inactive. This should be configured for future use (ex: have a variable that toggles it active and inactive and reflects the styles appropriately. Do not fully restyle the button every time we need to reactivate it.)
