Fix bug: A system should be able to fit the details even a small screen or when user restore down (minimize size) of the page

To Reproduce
Steps to reproduce the behavior:

1. Please login in the system using the link "https://icare-main.dhis2.udsm.ac.tz/" (username: admin, Password: Admin123).
2. Go to 'Clinic module'
3. Click on 'name of the patient'
4. Restore down or minimize size of a page of the system
5. Then, click on the provisional diagnosis 'type diagnosis' for example "Malaria"
6. See error, where the names of the diagnosis are not clearly separated even the patient demographic information's are not shown are hidden.

Expected behavior
A user expect that the name are clearly separated as when continue searching from the list and even the patient demographic information's are shown not hidden.
