Unknown Form Option on Survey filters not working correctly #4939

When you hit "Clear", the Unknown Form shows a count of zero, which is inaccurate - every survey (or option in the survey list) should always show the count of posts.
Selecting Unknown Form while no Surveys are selected should show posts only belonging to the Unknown Forms
