Require the format and processing of the table of contents

As raised in #93, there is a question whether the table of contents should formally adopt the content model and processing defined in the Publication Manifest specification.

We will revisit this in the next version of the specification.

Until this time, implementers of reading systems will expect a TOC format consistent with the one detailed in the publication manifest.
