This PR synchronizes use of BOOST_HAS_FLOAT128 to be similar to the use-cases found in other MP-backends such as cpp_dec_float. Some added provisions (for more constexpe-friendliness in the future have been retained.

Also added definitions for 128-bit fma in multiprecision/float128.hpp.
