Venn Diagrams of SMI/CPA patient registers

GPs maintain a register of patients with SMI (Severe Mental Illness) and the Mental Health Trust has patients with CPA (Care Pathway Approach). Combining the data together makes it possible to plot where patients are on both registers. For Nottinghamshire Healthcare NHS Foundation Trust this was originally done at a Trust level through a system called GP Repository for Clinical Care (GPRCC) and over time this request has expanded to PCNs and CCG level.
