The set up looks great!
I don't think the radio buttons are important, we can let design make the call there.
We did decide before to take out Name and Location to help remove blockers for the requester and make the form friendlier (https://trello.com/c/9I7Xiw9s). Maybe, we can change first and last name to just name (optional)? What do you think?

Just a start on the intake form. Very rough. Radio buttons are not native to SwiftUI, so I will have to find a way to do that. And did not yet integrate location services to enable the automatic location info.
