Summary

let AudioStreamHandler own its client listener lifecycle instead of returning unsubscribe callbacks
rename the disconnect lifecycle helpers to handle_disconnect() and shutdown()
centralize disconnect handling in daemon and TUI so reconnect cleanup is less ad hoc
