Cryptostreams 2.1

Minor fixes from 2.0 release:

• Fixed round-reduction of XTEA and MICKEY
• Disabled CryptMT as it fails to link in release mode on Debian
• Removed GPL code (with inactive CAESAR as well) and change licence to MIT
• Added contributing notes
• New generation strategy: decryption mode for block ciphers
