Allow users to download just one issue or pull request by its number instead of fetching all items from a repository. Uses GitHub's issueOrPullRequest GraphQL field which handles both types transparently.

Currently gh2md always downloads all issues and PRs from a repository, which can be slow and unnecessary when you only need one item. This adds a -n/--number flag that lets you fetch just a single issue or pull request by its number.
Usage: gh2md owner/repo output.md -n 42
It uses GitHub's issueOrPullRequest GraphQL field, so it works for both issues and PRs without needing to specify which one — they share a number namespace per repo. Everything else (comment pagination, markdown export, --multiple-files) works as before.
Tests were added covering the happy path, error case, and end-to-end integration. All 23 tests are passing.
