  1. Add slow cohort import spec 

    This spec attempts to upload a cohort that's 1000 records long.
Currently this is taking around 10 seconds to run as a spec.

  2. Speed up patient imports 

    Use .import to import patient and parent records in bulk, while skipping
Rails validations.
