Migration Guide:

• o-forms usage in any consuming app should use ^v5
• if you need n-ui-foundations in your app you'll have to include it directly within the app rather than relying on this component to include it

Remove use of flags for Users and Groups page links

Also removes the Licence Admin link.

• Change the "My Account" link to "Sign Out" link;
• Link the FT icon to the ft.com homepage.
