Always create a pk primary auto-incrementing key

This is a breaking change, and the create_pk and primary_key have been removed as no longer needed.

Now, a (hardcoded) pk AUTOINCREMENT INT will be created for every model. insert() will return a new Model if succesful which will contain the data and the pk.
