Problems

This is a problem oriented system. Each patient can have any number of problems. Problems are usually associated with a SNOMED concept id, but are not required to be associated with a concept id. Problems have multiple elements associated with them: control state, activity state, authentication state, problem history note, wiki notes, tasks, images, relationships between other problems, a problem timeline, a list of encounters where the problem was accessed, and a log of all activities associated with that problem.

Physicians, patients and other user types who are granted access to a problem can adjust subsets of the problem elements.
